Testimony and Photo Release Form
I understand the testimonial I have provided or am providing to Second Summer Nutrition/Byrd Schas was created at the request of and on behalf of Second Summer Nutrition/Byrd Schas, and may be published, distributed, and used in connection with promoting Second Summer Nutrition. I hereby irrevocably authorize Second Summer Nutrition/Byrd Schas as well as its affiliates, employees, agents, assignors, transferees, licensees, or any other individual or party designated by Second Summer Nutrition, LLC, the unlimited, irrevocable right to copy, publish, distribute, edit, and use in commerce the testimony I provided on 19/11/2025 in reference to Second Summer Nutrition/Byrd Schas for the purpose of publicizing Second Summer Nutrition, Byrd Schas, RD, or for any other lawful purpose. The testimony may be used on websites or by related affiliates, printed publications, other presentations, or in any other distribution media. I agree that I will not seek any payment or make any claim against Second Summer Nutrition, LLC for use of my testimony. I also waive any right to approve the finished testimony wherein my photograph, likeness, or words appear.
I also authorize Second Summer Nutrition, LLC to use, publish, and otherwise distribute my photograph, name, and city and state where I reside, all of which I provided voluntarily to Second Summer Nutrition, LLC for use. I agree that Second Summer Nutrition, LLC may use this photograph for any lawful purposes, such as publicity, illustration, advertising, and website content.
I hereby hold harmless and release Second Summer Nutrition/Byrd Schas from any causes of action, claims, or demands which I or anyone else acting on my behalf of on my place may have by way of this authorization.
